Dear www.future-fitness.com client It is now required that we have your signature on file before we can accept your credit card order. This serves as protection for us as well as you, our valued customer.
Please fill out the form below, and fax it back to us prior to placing your order. By signing this form you are authorizing your associates to sign for your package if you are not available upon delivery. If this is direct mail I authorize that the mail can go from post office to post office as long as I get a postal signature.  If a deposit is placed on the order, this form may also be used for the balance of payment.  Please provide us with your e-mail address below, so that we may send you an electronic receipt. WWW.Future-Fitness.com  will replace any items that are received damaged. The items must be kept in the original box, so that the carrier can issue a claim. A call tag will be sent for the returned items. There is a 30-day time limit from the receipt of the merchandise to let us know that you have a return issue. Items that cannot be returned are:

CUSTOM WORK: In the event that the customer is not satisfied with the work, Future Fitness International will make every effort to rectify the situation. We will not refund any fees imposed for design or printing set up. Once you have chosen a piece and it has gone to the graphic artist, it is considered a custom piece.
If the customer changes his/her mind, and the piece needs to be completely re-designed by the graphic artist, additional fees of $75.00 per hour will apply. These fees do not apply to any editorial corrections, only for new design set up. Additional fees are non-refundable.

SHIPPING FEES: In the event that any or all parts of an order are returned to Future Fitness International, the customer is held responsible for new shipping fees. These are also non-refundable. Replacement of defective merchandise is the exception to the rule, and replacement items will be shipped to the customer free of charge.

DIRECT MAIL:  www.future-fitness.com  is not responsible for the date of delivery of any direct mail postcards.  Post office receipts will be provided for each mailing.  www.future-fitness.com  does not guarantee any in-home delivery dates. If shipped directly to client instead of post office www.future-fitness.com is not responsible for lost packages of mailings.

Any disputes or enforcement of this agreement shall be bound over to the magistry courts of Broward County, Florida.  In case of dispute, the prevailing parties shall be entitled to attorney costs.

Business of approval. Direct mail can be up top days. Shipping used will be UPS / Fed Ex / USPO . Tracking numbers will be provided to customer.
